Lower back pain is very common. About 80% of people experience it at least once in their lifetime. For many, the symptoms return regularly.
The pain can start during physical effort, but also hours or even days later. Sometimes there is a clear trigger, and sometimes the complaint seems to come on spontaneously.
What causes lower back pain?
In practice, we see that lower back pain rarely has one clear cause. Most often, it involves a combination of factors, such as:
- Little variation in posture and movement, for example with lots of desk work
- Incorrect or one-sided strain, such as carrying a heavy bag on one shoulder
- Natural ageing of joints and cartilage (wear and tear or osteoarthritis)
- Tension, stress, fatigue, illness, cold or damp
- Changes in hormone balance, for example during pregnancy
- A fall or traffic accident
- An incorrect movement or twist, such as bending and turning at the same time
(for example lifting something from the boot of the car, gardening, or cleaning)
What happens in the lower back?
Because of these factors, a movement dysfunction can develop in the intervertebral joints (facet joints). When these joints don’t move properly, the intervertebral disc is loaded incorrectly.
This can overload surrounding structures such as muscles, ligaments, and nerves. As a result, pain nerves become irritated more quickly and the normal movement nerves function less effectively.
This may show up as:
- A nagging or stabbing pain in the lower back
- Stiffness or a “stuck” feeling
- Pain that radiates to the buttock or leg
You may notice that moving becomes more difficult or that certain postures make the pain worse.
